State Mandate for the State of Arkansas

1) Dr. Jerry Edwards, Dr. Tom Tvedten or Associate, is a Physician licensed by the State of Arkansas and will be performing your procedure.

2) According to the information provided by you, you are approximately __________ weeks by LMP or ___________ weeks since conception. However, on the day of your appointment an ultrasound will confirm the gestation.

  • The medical risks associated with carrying the pregnancy to term:
    • The risk of death and/or major health problems of the mother are greater than those associated with having a pregnancy terminated
    • Postpartum depression and psychological problems associated with hormonal changes
    • Uterine rupture
    • Embolism
    • Hysterectomy (Surgical removal or uterus and/or ovary and tubes.)
    • Infection (Post vaginal delivery or c-section delivery)
    • Excessive Bleeding (hemorrhage)
    • Stroke
    • Urinary and/or fecal incontinence
    • Permanent alterations in body shape and appearance
    • Pain associated with labor and delivery
    • C-section (Surgical delivery of term pregnancy)
    • High Blood Pressure
    • Diabetes
    • Death

4) The following are medical risks associated with the method of termination used by this facility (Vacuum Aspiration, Dilation and Evacuation or Medical Abortion):

  • Allergic and unexpected reactions to medications (Seizure, asthma, shock, aspiration of vomit)
  • Uterine perforation with injury to other organs (< 2 per 1000)
  • Cervical laceration
  • Excessive bleeding (hemorrhage)
  • Retained products of conception
  • Infection
  • Inability to terminate the pregnancy
  • Psychological problems post procedure (depression, grief, etc.)
  • Discomfort/Pain
  • Death (It is safer to terminate a pregnancy than to remain pregnant)
  • Hysterectomy (removal of uterus)
  • Infertility (inability to become pregnant in the future)
  • Embolism
